Community Health Needs Assessment

Cue the confetti! Harney County Health District’s 2025 Community Health Needs Assessment (CHNA) is now available.

This is the final product of a yearlong process involving a community survey, interviews with community partners, and the collection and analysis of data. The overall goal was to learn more about the health status, needs, and concerns of Harney County residents and develop goals/strategies to benefit the overall health of the community.

The 2025 CHNA includes Harney County Health District’s Implementation Plan. As part of the CHNA process, Harney County Health District (which is made up of Harney District Hospital and its affiliated entities) identified three priority areas: healthcare access/community outreach, behavioral health, and wellness. Goals and strategies for addressing these priorities can be found in the Implementation Plan on pages 11-13 of the CHNA.

In order to ensure that progress is being made on the Implementation Plan, reports will be presented to the Harney County Health District Board of Directors as well as the Local Community Health Partnership (LCHP) every six months and posted on Harney District Hospital’s website.

Whenever possible, Harney County Health District will seek to partner with local agencies and organizations to improve community health. In addition to collaborating on the goals/strategies outlined in the Implementation Plan, the Health District welcomes and encourages community partners to use the information presented in the 2025 CHNA to develop their own goals and strategies to support the health needs and concerns of the Harney County community.
