Help shape the future of healthcare in Harney County by taking the 2025 Community Health Needs Assessment (CHNA) Survey!
For the past few months, Harney District Hospital has been interviewing local healthcare providers and community leaders to learn more about the health and wellness needs of our community. Now, we want to hear from you! Your responses to the 2025 CHNA Survey will be used to set goals and priorities to help improve the overall health and wellness of our community.

Paper copies of the survey are available at Harney District Hospital (HDH), NewSun Energy & HDH Specialty Care Clinic, Symmetry Care Inc., the Burns Paiute Tribe’s Wadatika Health Clinic, Harney County Health Department, Harney County Library, Harney Hub, and the Harney County Chamber of Commerce.
To thank you for your time, you will have the option of entering a drawing to win one of three $100 Shop Harney gift cards, which can be used at numerous participating local businesses. After submitting your survey, you will be asked to enter your name and contact information. Identifying information will only be used for the purposes of the prize drawing and will not be connected to your survey answers. Three winners will be drawn.
